Aged Care & Home Safety Support

Supporting the over 65’s in connecting to their communities/family and to feel secure in their own homes!

‍ ‍Aged Care & Home Safety Support

We partner with aged care providers including Kalyra, ECH, and Komplete Care to deliver compassionate, respectful support to our senior community.

‍Our approach is gentle, flexible, and client-focused, helping people feel comfortable, supported, and able to maintain independence at their own pace.

‍ ‍Navigating My Aged Care

‍ We assist clients and families to navigate the My Aged Care system, including:

  • ‍Setting up and accessing the portal

  • Assisting with locating an Age care Provider in your Area

  • We do not have access to manage your funding or referrals

‍ ‍Support Through CHSP

‍We also support participants accessing the Commonwealth Home Support Programme (CHSP), working alongside providers to deliver services that align with each person’s needs and goals.

WHAT IS CHSP? - When Urgent Support Is Needed

‍ ‍In some situations, support can be arranged more quickly when there are immediate safety concerns. This may include:

  • ‍ ‍Urgent referrals through My Aged Care

  • Aged care providers prioritising support where there is risk, such as:

    • Falls

    • Unsafe living conditions

    • Returning home from hospital

  • Additional help from local councils or community services in more complex situations, including:

    • Environmental health risks

    • Severe clutter or hoarding

    • Community safety concerns

‍ ‍This helps ensure people can remain safe, supported, and comfortable at home.

‍ ‍Safe & Accessible Home Environments

We understand that homes can naturally become filled with meaningful belongings over time, or you may have had to downsize your home.  Our approach is always respectful and non-judgemental, with a focus on safety, comfort, and accessibility.

‍ ‍We can assist with:

  • ‍Improving ease of movement through the home

  • Reducing trip and fall risks

  • Supporting a clean and hygienic environment

  • Making it easier for carers and support workers to assist with daily tasks and personal care

  • Ensuring family members and emergency services can safely access the home

  • Preparing the home for a safe return from hospital

‍ We work alongside each person to protect the items that matter most, while creating a safer and more functional space.
